The mailing address and Web Site address for Life Insurance Beneficiary designation has changed effective June 1, 2007. The change is ONLY for Life Insurance.
It is important to maintain current beneficiary designations. Often the beneficiary is overlooked when life events occur. When this happens, benefits may not be paid the way the participant intended.
Most employees complete a beneficiary form at initial eligibility for Basic Life Insurance and when enrolling in contributory coverage�s such as, Optional Life and Personal Accident Insurance. However, when a change in family status occurs, the need to review and/or update beneficiary designations is often overlooked.
Family Status Changes:
� Marriage
� Remarriage
� Divorce
� Birth or Adoption of a Child
� Death of a Spouse, Dependent or Beneficiary
If you have experienced one of the above life events without reviewing your beneficiary designation, it is important to review and/or update your beneficiary designation. Please be aware that a divorce decree does not change your beneficiary of record. You must submit the appropriate beneficiary change form. This can be done any time, on-line, at https://mybenefits.metlife.com. For employees who do not want to designate beneficiaries on-line or are unable to view their beneficiary information on-line, you can request a new beneficiary form to be mailed to your address of record by contacting the GM Benefits & Services Center at 1-888-543-3461, or at https://mybenefits.metlife.com.
Life Insurance benefits are paid to the most current beneficiary of record. If beneficiary designations are not updated, life insurance proceeds may not be paid the way you intended. To ensure that your benefits are paid in accordance to your wishes, you are encouraged to review and/or update your beneficiary designation today.
