Modified Plan Cost � Single (TCN)
� Single members monthly cost is $11
� 2008 deductible is $159
� 2008 Co-Insurance is $106
� The $159 deductible is paid for the first of any medical.
� After the deductible is paid, the co-insurance is billed at 10% of the medical expenses until the $106 is paid.

� Family members monthly cost is $22
� 2008 deductible is $318
� 2008 Co-Insurance is $212
� The $318 deductible is paid for the first of any medical.
� After the deductible is paid, the co-insurance is billed at 10% of the medical expenses until the $212 is paid.
Prescription Drug Coverage
Retail - Generic $5, Brand $11, ED $16
Mail Order - Generic $11, Brand $16, ED $19
Emergency Room Co-payment - $53, waived if admitted
All PPOs and most HMOs are being eliminated for 2008.
